  • A Sworn Affidavit is a self-declared document in which the person affirms that all the information provided by them is true, correct and complete.
  • This affidavit is generally required for purposes such as obtaining a passport, visa, government job, or during any legal procedure, where an individual has to certify certain facts or circumstances, like giving consent for a minor or providing clarification when a specific document is not available.

Requirement of a Sworn Affidavit for Issuing a Passport to an Indian Minor

  • When parents apply for a passport for their minor child and one of them resides outside India, they are required to submit an affidavit.
  • In such cases, the parent living outside India must submit this affidavit.
  • It must be attested by the Indian Embassy or Consulate in that country, where the parent resides. The affidavit must carry the official stamp and signature of the authority.
  • Through this affidavit, the parents declare that they have no objection to the issuance of a passport for their minor child.
  • Along with the Affidavit, the parent living outside India must also provide a copy of their passports and the Annexure D form.
  • Along with these documents, the parent residing in India should affix a copy of Annexure C and submit the minor's passport application.
