Specimen Affidavit for a passport in lieu of lost/damaged passport (Annexure "F")

If your passport is ever lost or damaged, then while renewing the passport you have to fill Annexure–F at the Passport Seva Kendra, in which it is necessary to provide the details of the lost or damaged passport.
